Request for CIB and BIA 4432

Requests for a Certificate of Indian Blood (CIB) and/or a BIA-4432 Verification of Indian Preference for Employment require a notarized letter of request.

Submit your notarized request by mail, fax, or email to your Home Agency.

All mailed, faxed, and emailed requests are processed in the order they are received.

Notarized Letter Required

NOVRI observes the Navajo Nation Privacy and Information Act. Requests must include a notarized letter signed by both the applicant and the notary public.
