Request for CIB and BIA 4432

A request for a Certificate of Indian Blood (CIB) and/or a BIA-4432 Verification of Indian Preference for Employment must begin with a notarized letter of request.

The notarized letter may be mailed, faxed, or emailed to your Home Agency for processing. Please do not submit duplicate requests.

All mailed, faxed, and emailed requests will be processed in the order they are received.

Email addresses and phone numbers for each agency can be found under the Contact tab.

The Navajo Office of Vital Records observes the Navajo Nation Privacy and Information Act and therefore requires a Notarized Letter of Request. The letter must include your signature, as well as the signature and official stamp of the notary.
