Divorce Record Update

To update your Navajo Nation enrollment record following a divorce, please submit the required documents listed below.

Certified Court Ordered Divorce Decree
(Include all pages.)
Additional documentation
Your Home Agency may request additional information if needed.

Important

Divorce record updates must be submitted in person or mailed to the applicant's Home Agency.

Email requests cannot be accepted.

Notarized Letter Required

The Navajo Office of Vital Records & Identification observes the Navajo Nation Privacy and Information Act. Divorce record update requests must include a Notarized Letter of Request.

The letter must include your signature, as well as the signature and official stamp of the notary.

Full Name
Date of Birth
Enrollment Number
Alias
(If applicable)
Previous Spouse's Name
(If applicable)
Mother's Full Name
(Including maiden name)
Father's Full Name
Current Mailing Address
Phone Number

Important: Submit a complete, certified copy of the court ordered divorce decree, including all pages. Incomplete documentation may delay processing. Your Home Agency may request additional documentation if necessary.
