Updating Records for Divorce

To update your records to reflect a divorce, please submit the following:

  1. A notarized letter of request
  2. Certified Court Ordered Divorce Decree (include all pages)
  3. Your Home Agency may request additional information if needed

Important:
All divorce record update requests must be be submitted in person or mailed to the respective Home Agency for processing.

The Navajo Office of Vital Records & Identification observes the Navajo Nation Privacy and Information Act and therefore requires a Notarized Letter of Request. The letter must include your signature, as well as the signature and official stamp of the notary.
