To update your Navajo Nation enrollment record following a legal name
change, please submit the required documents listed below.
Required Documents

Notarized Letter of Request

Revised State-Issued Birth Certificate
Reflecting your legal name change.

Certified Name Change Court Order
Include all pages.
Important
All name change requests must be
mailed to the applicant's
Home Agency
for processing.
Requests cannot be processed without the required legal documentation.
Notarized Letter Required
The Navajo Office of Vital Records & Identification observes the
Navajo Nation Privacy and Information Act. Name
change requests must include a
Notarized Letter of Request.
The letter must include your signature, as well as the signature and
official stamp of the notary.

Important:
A revised state-issued birth certificate reflecting your legal name change
and a certified court order are required before your Navajo Nation
enrollment record can be updated.
