ServicesName Changes

Name Changes

To update your records to reflect a name change, please submit the following:

  1. A notarized letter of request
  2. Revised state-issued birth certificate reflecting the name change
  3. Certified name change court order (include all pages)

Important:
All name change requests must be mailed to the respective Home Agency for processing.

The Navajo Office of Vital Records & Identification observes the Navajo Nation Privacy and Information Act and therefore requires a Notarized Letter of Request. The letter must include your signature, as well as the signature and official stamp of the notary.

Information Needed:

     Reason for the Request
    (include the name of the document you are requesting)
     Full Name
     Date of Birth
     Enrollment Number
     Alias
     Spouses full name
    (current or former, if applicable)
     Mother's/Father's name
    (including mothers maiden name)
     Current address
     Phone Number
