Request to Relinquish

A request to be relinquished begins with a notarized letter of request, along with an eligibility letter from the other tribe, if applicable.

The notarized letter may be mailed, faxed, or emailed to your Home Agency for processing. Please do not submit duplicate requests.

All requests will be processed in the order they are received.

The Navajo Office of Vital Records & Identification observes the Navajo Nation Privacy and Information Act and therefore requires a Notarized Letter of Request. The letter must include your signature, as well as the signature and official stamp of the notary.

Required Documents

Original Birth Certificate
Return all Navajo Nation CIB's
Notarized letter containing Reason for Relinquishment
Name of other tribe(if applicable)
Eligibility Letter from other Tribe
(if enrolling into another tribe)
