Relinquishment Request

Individuals wishing to voluntarily relinquish their Navajo Nation enrollment must submit a Notarized Letter of Request.

If enrolling with another federally recognized tribe, an Eligibility Letter from that tribe must also be submitted.

Requests may be submitted by mail, fax, or email to your Home Agency.

Please do not submit duplicate requests. All requests are processed in the order they are received.

Required Documents

The Navajo Office of Vital Records & Identification observes the Navajo Nation Privacy and Information Act. Requests for relinquishment must include a Notarized Letter of Request.

The letter must include your signature as well as the signature and official stamp of the notary.

Please submit the following:
  • Original Birth Certificate
  • Return all Navajo Nation CIB Certificates
  • Notarized Letter of Request
    Include the reason for relinquishment and the name of the other tribe (if applicable).
  • Eligibility Letter from Another Tribe
    Required if enrolling with another federally recognized tribe.
Important: Relinquishment of Navajo Nation enrollment is a significant legal action. Please ensure all required documentation is included before submitting your request. Incomplete applications may delay processing.
