Request to Relinquish

Requesting to be relinquished begins with a notarized letter of request, along with an eligibility letter from the other tribe, if applicable.

The notarized letter can be mailed, faxed or emailed to your home agency to be processed. (Please do not send duplicate requests)

All mailed, faxed, and emailed requests will be processed in the order they are received.

Navajo Office of Vital Records observes the Navajo Nation Privacy and Information Act, therefore, requires a Notarized Letter of Request complete with your signature, the signature, and stamp of the notary,

Required Documents

Original Birth Certificate
Return all Navajo Nation CIB's
Notarized letter containing Reason for Relinquishment
Name of other tribe(if applicable)
Eligibility Letter from other Tribe
(if enrolling into another tribe)
